My husband was taken into the A&E by the Ambulance service on 04th April, I took a list of his medication. The dr looked at the list and I said that I would take the medication which may not have available at the hospital as my husband was kept in over night.

I went to the hospital the next day 05th April at 08:05am and asked the staff if he had been given the medication, I was advised that the staff didn't know what medication he was on, and I gave him the medication i had brought in.

The A&E trauma team did know what medication he was on but didn't seem to have commicated to ward 3.

The Dr faxed out GP for a list of his medication.

I am so sorry about this lack of communication. We work closely between A&E and Ward 3 (our assessment unit) and now have an electronic handover system which will ensure this does not happen again.
