My 83 year old mother was admitted to the hospital with severe pain and raised blood count level. She was given substantial doses of morphine and her blood pressure medicine which she has been taking for 20 years was stopped. On the 11th day of her stay she was discharged with 5 min notice. She was told (in front of me) that an assessment would be made of her needs would be made by "PACE" Luckily I was able to stay with her that night at her home, She is presently a frightened old lady stuck on her own with a climbing BP and suffering the after effect of Morphine. No contact has been made by PACE. I have telephoned today 4 days after her discharge to be told that no reference to PACE was made.

So basically the PRU chucked out an old lady on to the street with no check to see if she would survive without any medical backup
