(as the patient),

I was diagnosed with cataracts by specsavers who wrote a letter to my GP which I delivered that day.

14 days later a letter arrived form the surgery asking for my height and weight so that they can send of a referral letter to your good selves, which I did over the phone.

The next day one of your staff phoned me and made an appointment of my choice for me, your staff were excellent.
