When my mother who has diagnoses of dementia was admitted into hospital after a fall, she fractured her wrist and had pneumonia... My mother was declared medically fit to go home. The hospital wanted to discharge her back home but the family challenged that decision. Our family challenged the decision for our mother to be discharged back home we were then getting mixed messages about the discharge procedures. We were advised initially that there was no rush to find a care home for mum, but it became very clear we needed to look more quickly for a care home for mum... Once we found a home the assessor came and assessed that the care home could meet my mothers needs. Everything was done verbally there was no formal communication done between our family the care home or the hospital, other than to arrange a discharge date. I was disappointed with the process; I think the care home was inadequately prepared for mum. There should be a more formal procedure carried between our family the care home and the discharge nurse.

Thank you for posting your comments about your mother’s care and subsequent discharge to a care home. The Trust is committed to the safe and prompt discharge of patients from acute care. We recognise that appropriate, timely discharge planning is fundamental to the provision of effective health care and to the wellbeing of the patient. As a Trust we are also committed to ensuring that patients’ families and carers are involved and kept informed of all discharge arrangements. We are also aware that continuity of care is essential as patients are discharged from one care setting to another.
