"Treatment and care received during my wife's stay and subsequent death in hospital"

My wife died in Lincoln County Hospital in May 2012 aged 65. She had been under care and treatment for a blood cancer since October 2011. During that time and prompted by the extension to hospital mortalities enquiries following from the Mid-Staffs exposure I feel that her death was a premature one.
On numerous occasions during the various stays in Lincoln County, my concern was evident at the poor performance meted out to her. These include the delay and postponement in delivering chemo treatment drugs through insufficient qualified staff on duty and delays incurred almost every time the treatment allowed her to return home because of poor response from the hospital pharmacy. On numerous occasions, the ward staff was further depleted because of the absence of housekeeping personnel whose tasks seemed to be carried out by the nursing auxiliaries at the loss of their auxiliary duties.
There was a division of opinion between two specialities regarding whether my wife had suffered a stroke that remained unanswered, except by her death.
I've asked myself if this input is merely a reaction to having lost my wife and I strongly believe it is not and questions need to be asked
