"8 weeks after A&E and still no appointment for heart test"

About: Worcestershire Royal Hospital / Accident and emergency

(as the patient),

Discharged over 8 weeks ago after an overnight stay, following A&E admittance. Was told I would receive an out patients appointment for an echocardiogram.

Phoned several times and eventually told discharge notes hadn't come down from the ward and when they did eventually come down was told they hadn't been fully completed. Then told it had been a locum doctor who had dealt with me.

The notes appear to have been passed from doctor to doctor and today I was told that they are with a doctor for completion but it could be his junior who completes them and he has three boxes of notes to deal with.

I am still awaiting an appointment letter after more than 8 weeks!
