"Thanks for dr noticing I'd taken myself off anti depressants"

(as the patient),

when i started my anti-depressants; i thought i could cope so i took myself off them. i was lucky it was spotted by my doc. i was near topping myself. if it was not for my doc handing me to the crisis team i wouldn't be here, so i would like too say thnks

Good to hear you're feeling better. It's great that you had help like this when you needed it most.

Your story shows how important it is to talk about your feelings and your care with the people looking after you even though this can be hard. If anyone ever prescribes any medication for you they will want to hear how it effects you.

I will make sure your comments and thanks are passed on, as staff appreciate hearing positive feedback like this.
