"Poor review and monitoring systems"

About: Barnsley Hospital / Dermatology

On three separate occasions we have felt the need to raise concerns about patient safety in the Dermatology department due to a poor recall and monitoring/follow up process.Each time a quick fix is provided to alleviate the immediate issue but no changes are made to the processes that cause the problems and then the same issues arise at the next expected review date. Lessons are not being learned. Individual staff in the department are helpful and pleasant but are constrained by the system they are required to work within.
