"I think my Fathers death was avoidable"

(as a relative),

My father was re-addmitted into A2 after being told his infection had cleared up, when it had not.

It turned out he had septicaemia and I understand was not given the proper treatment, as it was not recognised. He sadly passed away in late February. I think the signs could have been spotted.
