I surrendered my driver’s license to DVLA. early 2018 due to surgery and a seizure. I had applied to DVLA to get my license back November 2018. West Oak Surgery received a form from DVLA to be completed and returned in order that I can get my license back.

This was received in your office December 12th, 2018. DVLA had said that within a week or receiving the completed form back from yourselves that I should have my license.

On this premise, I booked my holiday to USA for a wedding, we fly out on February 16th, 2019. December 18th, 2018 DVLA received a letter from the doctor, DVLA did not receive the completed form that was requested. I contacted DVLA and they had said that due to the fact the requested form was not returned as requested, my case had to be sent to supervisors to review. This process of course delays the processing of my application to have my license returned.

DVLA suggested I have your surgery complete the form and return it via email or fax in order that they could expedite the process.

When contacting your secretary staff, I was advised that doctor decided a letter was best method to respond to the DVLA request. When I explained the situation and asked that the form be completed and emailed or faxed to DVLA, I was told that DVLA would need to contact the surgery to explain why they required the form returned.

I am now in a position where I must cancel my trip to America as I must be able to rent a car. This means I will not be able to attend the wedding but also that I will not be able to visit with 96-year-old father who lives over 1000 miles from where the wedding is. This is all due to your surgery not following a simple request from DVLA to complete and return a form! I’m sorry but I cannot comprehend how or why a decision was made to not comply with a very simple request. Nor can I understand why, once informed of the issue that your surgery was in no position to assist in making this right. I have fully booked my trip that is costing us over £3000 of which £1800 is not refundable. Not only will I be out-of-pocket for these expenses, but I have had sleepless nights and ongoing high levels of stress that I am having trouble effectively dealing with and it’s effecting my work and home-life to no end.
