"Missing records"

I recently attended the practice to get a prescription for anti coagulants as i was travelling to the far east.

I'd been diagnosed 19 years earlier at APH with a blood clotting disorder that resulted in a late miscarriage.

Id seen a specialist haematologist for 5 plus years for regular blood tests throughout my next two pregnancies who advised my to take blood thinning injections to prevent DVT when flying.

When I visited the surgery and met with the Practice Nurse she refused to give me prescription for anticoagulants as nothing was in my notes about my blood clotting condition.

They had no records of the blood disorder, no record of my two subsequent pregnancies or my appendix removal !!!

The admin staff advised that a lot of the old paper records had gone missing during the move and switch over to electronic format.

A whole 5 years of medical notes where missing and no one seemed to be interested.

I was left to chase APH to get confirmation of my diagnosis and they subsequently sent the confirmation and missing info back to the GP surgery before i was eventually given a prescription.

Appalling service.
