About: University Hospital Wishaw / Care of the Elderly (Wards 9-12)

(as a relative),

Elderly father discharged from hospital with no carers arrangements made, no Warfarin dosage advised, In fact no instructions of any kind issued. Still suffering confusion caused by UTI which was still being treated.

Sent home to his own home in the care of my Elderly and extremely frail mother. We had been advised he would get home at least 24 hours later than he actually did so we were unprepared, the agencies who should have been involved were all closed for the evening. Totally unacceptable.
