"Wrong date of birth"

A relative recently discovered their date of birth was incorrect at Theydon Bois surgery. In spite of asking it to be corrected at Theydon on two separate occasions, on an emergency admission to a and e at Enfield it was discovered that it had not been corrected at the Theydon surgery, meaning medics were unable to do a chest x ray in case they had the wrong patient. A couple of days later the relative had an emergency admission with pneumonia. On going back to the surgery myself, it was discovered they still had the wrong date of birth which they very apologetically altered there and then.

I feel this mistake could have had very serious consequences and am concerned it took three interventions to get it amended
