"Outpatient visits"

(as the patient),

I have had several visits recently to see various consultants and to have tests. Everyone on the reception desks and in various departments has been friendly and kind. One of the nurses was very helpful when I was struggling to go round to the MRI department due to my balance issues. As a nurse myself I have found it really hard to be so unwell and I really appreciated her kindness. 

Various consultants have been helpful and thorough and I appreciate all the help I have received from their secretaries too. The only small point is that one of the consultants does not routinely copy the patient in to GP letters (the others did) and also they did not write to my GP after my follow up appointment.

This was a bit disappointing as the GP was waiting for some information before making another referral on the basis of this. I did feel a bit let down by this.

I am very pleased that you found our nurses to be so kind and our Reception team to be helpful.

I am sorry to hear, however, that one of our consultant's was not as communicative with you and with your GP as they could have been.

It would be very helpful to know which consultant this was so we can feedback to them; if you wouldnt mind bringing this to my attention, I would very much appreciate it.
