"Disappointing levels of communication and safety concerns"

About: Musgrove Park Hospital (Taunton)

(as a relative),

My father spent 2 days on AMU at Musgrove and I was very disappointed in the level of communication from staff. I had to constantly ask to speak to doctors and nursing staff and in actual fact didn't mange to speak to a single Registered Nurse during his stay.

One of the junior doctors did their best to tell me (with my dad and mum) what had happened - but they were not looking after him so only had his notes to refer to. I was told that I could only speak to the senior medical team if I was on the ward when they did their round - impossible when I'm working! His 'named nurse' was always busy doing something else.

His IV fluid chart stated 'wrong fluids given - changed'. This to me is a medication error - and even with no consequences, it should have been pointed out to us and investigated - not to blame anyone, but to learn from it.

This admission contrasts greatly with a previous admission in June to CCU where the care and communications were exemplary.

I have 40 years of experience working in and with the NHS as an RN, manager, etc (including patient safety) so absolutely understand the challenges, but this was not a good experience.

Dear Concerned Daughter 2017

Thank you for posting your story on the Care Opinion website. I was very sorry to read of the communication problems you experienced whilst your father was an inpatient on the Acute Medical Unit. It is very upsetting as we do try as a team to be as visible and communicate well with all relatives and patients on the ward. I do hope that your father is continuing to recover well.

I would welcome the opportunity to discuss this in more detail with you as I feel that your concern warrants a comprehensive and thorough investigation to prevent future issues. I would be grateful if you could email me with your contact details so I can call you at a convenient time. My email address is sharron.syvret@tst.nhs.uk.

I look forward to hearing from you soon.

With kind regards

Sharron Syvret

Ward Sister
