I have been attending this department for 18 months. In that time, my notes have been lost twice, my identity not even recognized by the computer once and information requested by the DVLA, delayed by six weeks.The Consultant has never taken a history and has misquoted it multiple times. A simple five minute test relevant to the symptoms I described was not done so missing a critical clue for a second diagnosis. This was only followed up when I wrote and requested a review. The next test showed the second diagnosis, which needed rapid treatment. The referral letter was not sent for over a week. As a result, the second diagnosis has resulted in some permanent loss of sight. I have had no useful treatment for my initial presenting diagnosis.
Dates are misleading. I have had numerous appointments from April 2016 to September 2017.
