"Very disappointed with the care provided- west..."

I am a family member of a patient that has recently been discharged. I have experience of mental health facilities in other cities and this one very much fell short of any expectation. I was disappointed in regard to lack of communication!

My mother was terminally ill with cancer and however unfortunate this is, i feel it seemed to create a huge issues of what to do with her as other doctors had decided to remove her mental health medication and with no real comprehension that this would be the end result. I am tired of trying to fight to get any sort of cohesion care for her. I am sure if you asked her before she was heavily manic if this is what she wanted to be remembered for that the answer would have been Never.

Upon discharge which was obviously rushed due to the cancer diagnosis, after being reassured that they had contacted everyone. I have since found out that there was no district nurse care for the first two days - that ment that she has missed two doses of her insulin and a life threaten blood thinner medication to stop further blood clots being created. How are you ment to have faith in anything when you have been reassured of anything being completed to the standard required then you find this out later.

I was told on discharge that she is going to have support in the community and they will monitor this as they said she was no longer acute, but still not mentally well.

Her safety is now at risk as being demonstrated by the phone call i received Monday stating that she was now missing and the police are looking for her. She has been gardening outside the front of her house wearing a pair of pants and tshirt-as an elderly lady with terminal cancer this is going to kill her alone.

She has been spending £1000s in the 2 weeks since her release and i believe she is no longer sleeping again and this will only increase the speed at which the cancer is going to affect her. I have received calls from the care team to express concerns as they are barely seeing her at home even in the bedtime calls. Where pre-mania she was going to sleep a couple of hours before that call. I am not having any response from leaving messages with the CPN or Social Worker.

To increase the stress and concern for this case My mother fell out with me due to the dr telling her in the morning that she can have S17 leave with family that day, however no one contacted me, plus the doctor obviously had no concern to look into her history of absconding as she would not have come back. So when I arrived at visiting hours on my birthday she wants to leave and go home to bed. I try to explain to her that I can take you out if you wont agree to come back as this is only leave for a few hours not overnight. She was still so manic that this was not being taken in and she would not listen. I went to staff to get some assistance they stated if she isnt going to come back she cant go. Consequently she is now refusing to speak to me because i would not remove her.

I am sorry to see that we have failed to meet your expectations as an organisation. We are obliged to work alongside data protection laws, and have limitations placed on us that should a client withdraw consent to inform family members we have to respect their wishes. I am sorry that there have been difficulties since your mother has left hospital, and hope that these are resolved.

South Staffordshire and Shropshire NHS Foundation Trust have structured community pathways in place in order to try and maintain a person’s wellbeing at home and treat a hospital admission as a last resort. We have the requirement of following up a person discharged from hospital within 7 days, and assessing the support required from there on.
