About: Highbury Hospital Nottinghamshire Healthcare NHS Foundation Trust / Adult Mental Health Services - Community (County)

(as a service user),

my care plan was never put in place

discharged without any care plan

no comunication between my mental health services cpn

and hospital

mental health team never came to my reviews

out for 2 weeks and still waiting for care plan and left in limbo
