"No communication between departments"

About: University Hospital Wishaw / Emergency Department University Hospital Wishaw / General Surgery (Wards 16-18)

(as a service user),

Mine is a rather long story but I am at my wits end.

In May last year after attending my GP on numerous occasions and being admitted to Wishes General with SEVERE abdominal pain, I had an endoscope done and was diagnosed with Gastritis.

In June the pain got so SEVERE I was admitted by ambulance, bloods showed my liver function was poor. I was given pain relief and anti biotics by IV. I had an ultra sound done which showed a stone in my bile duct. I had an ERCP done but it was unsuccessful, I took pancreatitis and was in hospital for 11 days.

In July I was admitted by ambulance on 2 occasions with the same SEVERE pain, my liver function was poor, I was jaundiced and was given anti biotics, anti sickness and pain relief by IV.

In August the pain was getting more SEVERE and I was admitted again, I had emergency surgery to remove my gallbladder and was later discharged with no follow up instructions.

In September the pain returned and I was admitted to ward 18. An ERCP was done under General anaesthetic to try and remove the stone from my bile duct. Again this was unsuccessful as the stone was measuring 2cm, I had a stent fitted and was put on the waiting list to have the ERCP repeated.

In October I was taken by ambulance. On one occasion I was given morphine, anti sickness, anti spasm and anti biotics by IV in A & E before being sent to a ward, . On the second occasion I was told obviously it was the stone, I'd to keep taking my meds and they sent me home.

In November I was taken to day surgery and successfully had the stone removed under General anaesthetic.

In February this year the pain came back even more SEVERE and I was admitted again. Bloods tests were normal, I had an Endoscopy done followed by an MRI AND ultrasound. It showed that my bile duct was measuring 11mms. The consultant who removed the stone said it would suggest there was something wrong with the bile duct and he would look over the notes and get back to me. I was discharged without any follow up.

A week later I was admitted with the same symptoms and had a Ct scan. I was discharged with dihydrocodeine and oramorph to take at home to ease the pain. I was assured that a referral would be made at the consultants clinic and I would be seen within 6 weeks.

Since then I have been taken by ambulance On several occasions to A & E as the pain is now chronic. My supply of oramorph is finished and my gp won't prescribe me any more.

The care varied on these occasions from being given pain relief on arrival with anti sickness to being left crying in the bay with the pain, , being patronized by the registrar on call, telling me I'd be better at home as it was the weekend and nothing would be done on the ward anyway, his assurances that he had taken my details and was going to chase up my referral and being discharged alone after having morphine at 1am to make my way home, to being told to take all medicines regularly whether I needed them or not and hopefully that would stop the pain.

Recently the pain was so SEVERE I was screaming out, paramedics were called and had to give me Entenox to even move me from the house to the ambulance. I arrived at triage in wheelchair, still crying as the entenox had worn off. The triage nurse didn't see me but told the paramedics to put me in the waiting room. After a wait I was finally taken to triage where the nurse said I should have gone to out of hours, she gave me some mild pain relief despite the fact I told her I'd taken strong medicines which hadn't helped at all. She informed me she didn't know what they would do with me as there didn't seem to be anything they could treat, and I was sent back to the waiting room still in pain. I had no privacy, I was rocking back and forward and moaning out loud, the pain was unbearable. After a long wait I left hospital without being seen, by anyone as I needed to lie down and I was being sick.

Today I called the consultants Secretary to enquire about the referral 3 different doctors assured me had been made, she searched and told me there was no referral for any of his clinics. I feel very vulnerable as I am still in pain even as I write this, I have been made to feel like I am seeking attention and there is nothing wrong with me. Where do I turn, who is going to help me?

Dear Kdsmum,

I was sorry to have to read your negative experience within the emergency care areas within the hospital.

I am also concerned that you felt that the care that you have received was not person centred or effective and that there is an unclear plan for the future management of your condition.

I would be grateful if you could give our patient affairs team a call to provide me with more information in order to provide you with a targeted response.

They can be contacted on e mail at Yvonne.ross@Lanarkshire.scot.nhs.uk or via the telephone on 01698 366558.

I hope this finds you feeling better and hope we can work towards providing you with a detailed response to your questions.

Best wishes

Andy
