"Wrong medication from Pharmacy"

About: Aberdeen Royal Infirmary / Pharmacy

(as the patient),

As a patient, I was prescribed Peg interferon 90mcg by my the pharmacist on the outpatient ward 307 (haematology).

I took this script down to hospital pharmacy (on site). When the medication was handed over the pharmacist said I would have to reduce the dosage to 90.

I didn't think anything else of this until I looked into the bag (at home).

I was giving 2 x PeGASYS 180mcg Proclick™ single-use autoinjector (this can't be reduced to 90mcg). I tried phoning pharmacy but phone went straight to answer machine.

When I return to the pharmacy they admitted to their mistake But then made another mistake, giving me the correct medication (syringe) without needles. I pointed this out and the issue was resolved.

This would have been dangerous and made me very ill if I had injected the full medication.

I have shared your comments with the head of acute pharmacy and the lead pharmacist for that department.

I am very sorry there was a mix up with the dispensed dose and the information you received. We take this very seriously and have reiterated what should happen with the dispensing team. I'm sorry you were not given clear instructions that thankfully did not lead to a more serious issue thanks to your own vigilance.

The department wanted to pass on their own apology and have taken on board this shortcoming.
