I was impressed to see that my relative has been given a care plan to help everyone make good decisions about her health but was amazed to see how many times her name had to be hand written on the plan. We gave up after we counted her name handwritten 90 times. Yes, I promise you, 90 times for just one patient! Surely someone else must have noticed that this is a waste of someone's precious time?
