"I am concerned these notes will affect my ongoing treatment"

(as the patient),

I am sat at home reading my blue notes. I attended A&E a few weeks ago with ongoing pain/temp due to UTI. I was asked by GP to attend.

The medical notes are quite shocking to read. The obs recorded are incorrect. The use of language by Drs is not factual & a doctor questions if I actually spoke to infectious diseases in the notes. The notes also said I refused treatment which is inaccurate.

The senior Dr who I saw later that evening was great. Her notes reflect her professionalism.

I am left feeling upset that I have been misrepresented as a patient. I have not had my baby yet and I am concerned these notes will affect my ongoing treatment.
