"Treatment and discharge from hospital"

About: Tameside and Glossop Integrated Care NHS Foundation Trust

Family member admitted to Tamside on arrival via ambulance. Assessments carried out found patient suffering pneumonia. Initial assessment very good. Bed became available late afternoon of same day. Antibiotics administered via drip and nebuliser used to open airways. Transfer to MAU, care started to decline, missed morning antibiotics, staff tried to cover up error. Meeting with junior doctor to discuss the case, agreed assessment required before discharge from hospital. assessment to include stairs. Agreement ignored. Patient sent home. Patient unable to climb stair to bathroom. Patient in difficulty at home cooking a meal. Patient sent home with no help. Patient 80yrs old and lives alone. Contacted ward for explanation for discharge from hospital. assessment deemed capable to manage. Conversation then recommended an ambulance call to re-admit to hospital via A&E and to be recorded as failed discharge! Very poor patient care, conflicting reports.
