"No Discharge in Place"

As a family member and carer of a person going into A&E at Royal Oldham Hospital with hyperthermia.

On discharge no care plan was discussed or put in place, and the patient had to make their own way home.

This lead to phone calls to me some 2 hours after discharge as the patient had still not arrived home and took over 5 hours to make her way some 4 miles away.

This made her condition even worse and to date I feel like we have been left to pick up the pieces where I believe there clearly should of been a care plan in place.
