My dad was admitted from care home with chest infection he has dementia which worsens when I'll.

He is catheterised and on several family visits we had to ask for water jug or cup which had been removed and not replaced by staff. Also found his bed table pushed away from bedside out of reach. He was discharged with pressure sores to his bottom and this information wasn't passed onto care home staff on discharge. After being on trolley in AE unit for 24hours he was put on ward family came in and found him in bed on ward still wearing day clothes 48hours later, doctor informed and asked for dad to be cleaned and changed as he had odour.

Moved to rehab 24 hours then discharged to home no notification to family despite requested in file on each ward came in by 2 buses and he had gone.

Re admitted to Chorley less than 48hours later, on call doctor called by home.
