"My mothers records ( and my fathers ..."

What could be improved

My mothers records ( and my fathers instruction on her admittance) could have been checked so she wasn't given medication to which she has a known unfavourable reaction!! She came in in a good state but a week later has deteriorated due to this basic error. This is extremely worrying and distressing -not to mention totally unsatisfactory-- we have to trust doctors and nurses and this incident certainly doesn't make that easy.So communication could have been massively improved.

Anything else?

Surely a hospital has a duty of care. Information provided by next of kin should be noted and acted on otherwise why bother asking questions in the first place - notes need carefully checking. It's interesting that the care rating is only FAIR whilst financial management is EXCELLENT- surely in a hospital EXCELLENT should be what is achieved for care.The above incident is basic and avoidable if jobs are done diligently and correctly.Not impressed.Be diligent if you are a next of kin, keep asking questions and check what is going on.
