"My lovely fathers end of life care"

About: Nottinghamshire Healthcare NHS Foundation Trust / Mental Health Services for Older People – Community (County)

(as the patient),

my father was a 92 year old man with dementia. In April this year, I realised there had been a drastic change in his health. Dad lived with me in a self contained annex for the best part of a year and my promise to him was that I would do everything to keep him with me to the end. This is where Joanne, from the community health care team along with other wonderful nurses in our area stepped in and made my promise to my dad become a reality. Jo and an ever widening team, where there for my dad and me and supported me throughout the very daunting process of Dads death. I believe we in Mansfield are very very lucky to get the absolute care, compassion, empathy and support which was over and above excellent. Day and night I was thoroughly supported, helped loved and cared for! ! ! ! I cannot thank the community care team enough for myself, my dad and my family

Thank you for taking the time to give the feedback at what must be a very emotional time for you.

Losing a loved one is so difficult but to feel as though, when the end came, it was as as filled with love as is possible helps the grieving and healing process.

I am so happy you were able to feel supported and helped through your final weeks with your lovely father.
