"Excellent care , let down by the Clinical..."

About: King's College Hospital (Denmark Hill)

My elderly slightly confused husband was sent in urgently to the A and E as a possible stroke .Excellent care in that unit . Thankfully no stroke .Dr there discussed changing meds from warfarin to a newer anticoagulant .Their words 'He is being kept in overnight and will be seen by a multidisciplinary team tomorrow , including a physiotherapist , and they will sort out the change of anticoagulant . Unfortunately this was not the case .He was taken off a Bp medication , and another one . No mention of change of drug .Discharge summary vague and fobbing off back to Gp .GP cant prescibe these drugs , need hospital to initiate , now we need a referral , and a possible 2 month wait , while an elderly confused man cant manage his warfarin . Drs should be aware of systems in other parts of the hospital The left hand doesnt know what the right hand is doing !.
