"Re visit to out patient app on 28th June"

About: Derriford Hospital

We were very concered when we saw a consultant regarding a follow up on a CT scan and a previous app on 12th april they had no idea regarding my husbands previoue medical history or the CT scan or my husband Arterial Vascular problems.In fact they thought I was the Patient. They then realised they had the wrong patients records.

I think the department should look at there admin procedures, as if this had happend in an opperation situation it could be very serious. At no time was my husdand asked to confirm his name, datd of birth, and address. The consultant was a very nice person and we had a good consultation in the end. Our confidence in this department is very low.
