The team became involved in a time of crisis of a family member after a referral from Alcohol and drug abuse support team at my request. They immediately supported the patient and continued to do so after his discharge from hospital. Over a period of several months they assessed his condition and put a care plan in place to aid his recovery and they always made me feel part of the process. The team have worked hard with the patient and listened to his views on the level of help he would like and have now handed his care over to Housing support and this seems to be working well. The patient has recovered remarkably well although he has a degree of brain damage now he is grateful as am I for the invaluable help from this team of professionals.

What a truly inspiring story this is. It goes to show the great progress that can be made where there is willingness on the part of the patient, support of a loving family and commitment by a team of professionals. These are complex situations requiring careful planning and it is great to hear how staff listened and involved you in the process.
