"receiving a letter for my husband"

About: Luton & Dunstable Hospital

(as a relative),

I received a letter ddressed to my husband to complete an IBD Registry Consent Form to collect information into Inflammatory Bowel Disease. At the top of the form was his Patient Number. It was very upsetting receiving this letter today as my husband passed away in November 2012. I do hope this error is addressed as I do not want to receive anymore communications from the Luton & Dunstable Hospital regarding my late husband. Perhaps you would pass on this information to the relevant department
