"Lost DNR in A and E"

About: East Surrey Hospital / Accident and emergency

(as a relative),

My Dad collapsed at home and was bought to East Surrey Hospital by Ambulance. The paramedics were given our copy of my Dad's 'DNR'. Fortunately my Dad was discharged from hospital later that day, but when we got home we realised his 'DNR' had not been returned to him.

After numerous phone calls to A&E they are unable to locate this important document and have advised us to contact the Ambulance service ourselves to see if the paramedics who bought my Dad to A&E can remember what happened to it! 

It was a very distressing decision for my Dad to have to make, to complete a DNR and this is totally unacceptable. It's such a shame, given that he reports that care he received whilst at East Surrey was very good!

I am very sorry to hear that your father’s DNR was misplaced whilst he was a patient in our emergency department and I can fully understand the distress this has caused.
