"Discharge from hospital"

About: North Cotswold Hospital

My mother-in-law was in the hospital for 5 weeks after suffering a mild heart attack. We had one meeting with the social worker, and head nurse who explained she'll be discharged once they've put together a care plan for when she's home.

Gave us a name of an independent living home we could contact as we were concerned on her living at home with limited mobility.

We didn't hear anything further then 3 weeks later received a call from the hospital to tell us she'd be coming home in 2 hours.

Explained this wasn't sufficient time for us to get things ready for us as no prior warning had been given.

Called the hospital and asked why they hadn't brought her home first to assess if she would make a cup of tea, get to the toilet etc to be told it was passed this now and she wasn't able to walk. This was the first we had been told she was no longer mobile and would now be bed-ridden at home. My mother-in-law was then brought home the following day by ambulance; we were told we'd get a call once this had left the hopsital to give us an hour's notice so we had time to leave work and be there when she got home. This didn't happen, we received a call to confirm they were outside of her house but didn't know how to use the stand aid to get her into the house, did we know how to use it and come show them?

We hadn't been shown and I believe they picked her up and carried her to her bed.

I've spoken to several people at the hospital about how poorly my mother-in-law has been treated and how little support we've been given on supporting her. The care plan she's been provided is insufficient, she has to go 12+ hours between visits from her 'carers' to put her onto the toilet, get her something to eat and maintain her medication.

4 days later, I called her GP for a home visit as I was concerned about her health as the following day being home, she'd banged her leg in bed and had a nasty gash on her shin. Worryingly, they hadn't received any confirmation from your hospital on the care plan for her, and hadn't actually been advised she'd had a heart attack and had been in hospital.

Her son and I are doing our best to care for her but we're both in full time employment and have no experience with dealing with this situation so the stress and anxiety we're feeling is massive and I can't begin to explain how let down we have all been by you.
