"Your patient Valerie Howe needs urgent review..."

About: Whitchurch Hospital

Valerie has been told that she will be taken home tomorrow at 2pm where she lives alone and cannot climb the stairs to the loo! She only has one friend nearby, a neighbour Alan who visits her daily. Her walking after her operation is very poor and she shows signs of confusion. Your staff seem to have ignored all these facts and are about to condemn her to a dangerous situation, very likely a fall down the stairs. If this happens another NHS bed will be occupied for some time.

Action is required now.

Many thanks for taking the time to write to us. I appreciate your concerns for Valerie and understand the reasons for writing to us. We take all concerns very seriously and try our best to support patients and family by improving services for patients, wherever possible.

I would like to assure you that a thorough assessment is conducted by the discharge team before a patient is discharged either back to their own home or to a nursing/residential home. Patients can only be discharged when the doctor decides that the patient is clinically fit for discharge.

Hope you find this useful. However, if you continue to remain concerned, I will be grateful if you are able to contact me on 01743 277689 or 0800 032 1107 so that I can take all the necessary details and seek Valerie's consent before looking into this for her.

Thank you once again for taking the effort to write to us.

Kind regards

Soma
