About: Northwick Park Hospital

11.8.2015 at arround 12.30 I fell down injured my left hand , dislocatted from shoulder

Went to NPH in ambulance. Had to stay over night to be treated by specialist. Next day morning all was set up and discharged on 12.08.15. I had good service and staff were co-oprative. On leaving the hospital I asked for discharged letter but was told they will send me/ GP. I have not received the discaharged letter till today 19.08.15 so called the NPH A.E Dept.

tel 0208 8692028 passed to various department non of them have any record of my visit and so discharged letter,. Talked my GP they have not received any letter or information of my accident or discharged letter and my next appoiment for check up / treatment

I am in pain. Whom should I report?

I was very sorry to learn of your recent visit to the A&E department which necessitated a night's stay for treatment.

I would first of all like to thank you letting us know how pleased you were with the care you received during your admission, your kinds words are very much appreciated.

I was however concerned to learn of the difficulties you experienced in obtaining a copy of your discharge summary.
