I re-engaged with Sussex partnership nhs foundation trust in May 2012

I was put on a 6 x1hr course

Two weeks after the course finished my care coordinator wrote my care plan - retrospectively, with factual errors. She did not seek my input, contrary to CPA, She refused to include my thoughts. This was Aug 2012. She did not tell my GP that I was with services, she did not send him a copy of my careplan - although GP requested.

She said I'd get a new care co-ordinator when I went for my next stage whatever that might be - she had no idea - and I'd get a new plan and we'd review this one.

That didn't happen.

She wrote the next plan in draft - carrying forward the same errors. She flatly refused to change the plan to include my thoughts.

She did not firm up the plan - last I knew it was in draft (2013)

She did not send even the draft to my new therapist (even by the completion of a 10 week course in March/ April) and she still did not send me a copy.

In the middle of February she agreed to remove the statement that I enjoyed the 6 week course - the course was inappropriate to meet my needs

By March 2013 she had not sent my GP a copy of either plan, even though I'd been in services since May 2012.

In the end - after repeated requests - her manager sent the plan. Still with the errors in it!

I would like a corrected version of my care plans

I would like apologies

I would like corrected versions of my plans sent to my GP for his records

I would like SPT to acknowledge that CPA was not followed in either plans

I would like SPT to apologise to my GP and to me for not keeping my GP informed.

Hello Emma,

Yes, of course I raised this at the time.

Indeed, (as stated on Patient Opinion) it was my care co-cordinator's manager who eventually stepped in to send the two plans to my GP. Regrettably they were still in draft, and therefore unsigned. In other words, the line manager, himself, sent draft, unsigned, inaccurate information to my GP some six months after my GP was asking for information about the care you were providing. I had already been in your care for four months, but my care co-ordinator didn't tell my GP that either. This is in eCPA and also my GPs notes.

I am grateful, Emma, for your intervention now. I agree with you - these events should have been brought to a sensible conclusion long before now. It is a great pity that my care co-ordinator continued to refuse to add my thoughts to / amend the plans; that she broke her personal commitment to my GP to send the plans albeit ten months late; that her line manager sent draft and unsigned plans to my GP
