"End of life what time ? A and E shambles"

We lost our dad In June in your A and E department ... My sister rang to find out how dad was not long after admission not at any point was it brought to our attention how I'll our dad was ... She got there to find out he had passed away ... No great explanation ... Wasn't until later on when we asked for time of death we were told was 12.02 but my sister had arrived it then week or so later was 12:10.. When being collected out funeral care director was told He was sent home on date he died half hour later than the first time he apparently died .. Does anyone know any time last moments... How do you lose people for a while and mix up such sensitive information that he was alive but dead ... Mind you in March 2005 you told me my daughter and I were HIV positive for a week after I gave birth no was faulty machinery ruined a week of my life ...we tried to seek further help on that no one did ! At what point does one stop cocking up ?
