"3 referrals - 3 lost referrals"

My wife was referred to Castle Hill - suspected breast cancer - referral lost. My wife was given an urgent referral for urinary tract stent change - referral lost. My wife was given a referral for a CT scan -referral note incomplete therefore ignored by radiology department. My wife was agmitted to Acute Assessment Unit - supposed to be there only overnight - left there for three days. Department described as fikthy, horrible dump. We complained to PALS (Patient Advisory and Liasison Service) who promised to call back. Call not received. OIn order to gain admittance to this hospital we have had to contact our MP.
