"Consultant Letter to GP"

About: Royal Alexandra Hospital / Rheumatology

(as the patient),

I asked for a copy of my consultant's letter to my GP regarding some test results. On reading the letter there is alot of wrong information and it is as if the consultant has got me mixed up with someone else. The medication is wrong, family history is wrong and my medical history is wrong. The lifestyle section is a total joke. He has put me down as drinking excessive amounts of alcohol per week. I don't drink alcohol due to my medication and l am not a drinker. The overview section is different from what he told me at me last appointment. I really do not feel comfortable dealing with this consultant anymore due to this and l now understand why at my last appointment he was not at all interested in anything l had to say. He dismissed me and made me feel l was wasting his time. He has mixed me up with another patient of the same name. This has happened in the past at this hospital when l was expecting. That is how l know someone has the same name as me, same month and year of birth. I wish to ask for a second opinion or change consultant now. Consultants should be more careful with patient data and check details. The next letter should make interesting reading.

I work in a small team in NHS Greater Glasgow and Clyde which seeks to involve patients and carers in the work of the NHS. It is my job to give our patients and carers the opportunity to give us feedback, and to make sure that this is passed to the right people to help us improve the services we provide.

It is worrying to read this and I would appreciate if you could contact me as soon as possible to resolve this for you. I recall your previous post with regards to the experience you had with your consultant and would like to get to the bottom of this.
