I have mental health problems and moderate learning disability.

I had a appointment with a doctor for a check up, he didn't give me check up, just sat there and read out which doctors I've seen and what it said on his computer, ask him if he could fill in a form for a voluntary agency, he asked me for money, I didn't give him any so he wouldn't fill it in or sign it, he said it was a private referral so he would have to charge me.

He also didn't read the learning disabilities health book that I was told to fill in.

When I made a compliant about him, he wrote out a letter saying he could recall a discussion regarding a referral form but he understood I have since discussed this with my usual GP.

I have not shown my usual GP a referral form for a voluntary agency.

I have tried to talk to the manger about this and don't feel like it was helpful.
