About: Telford referral and quality service (TRAQS)

(as the patient),

I had an eye test at boots opticians Telford Centre. My eyes were photo x-rayed (1st time) and the optician noticed some irregularities. He sent a referral letter to my GP (Dr Hart Woodside). Dr Hart then made the referral request to TRAQS.

TRAQS called me on my mobile we agreed on an appointment. I attended appointment.
