"Hospital Admin"

About: East Surrey Hospital / Older people's healthcare Surrey and Sussex

(as the patient),

It has been almost a week since I left the hospital and yet my GP still has not received the documents the hospital said they were going to send over to him. These documents are very important as I am unable to leave the house without feeling dizzy and ready to faint again. these documents can help my GP recommend some sort of treatment.

Dear Kamin

Thank you for getting in touch with us. We apologise for the delay in forwarding your paperwork, but we have been experiencing some difficulties with our discharge summaries. These issues have now been resolved.

In order for me to look into this for you and to find out when the documents will arrive, please can you forward your full name, the date you were admitted/ ward name and your GP's address to maxine.may@sash.nhs.uk.

In the meantime, if you are continuing to feel dizzy then please contact your GP for advice. However, if this continues to get worse then please do not hesitate to come to A&E.
