"My father in laws last days in hospital "

My father died at Manchester Infirmary following a catalogue of failures of care in the community from his own GP and the care workers assigned to his care at home. Being terminally ill in addition to his ongoing health issues the GP failed to provide adequate support and district nurses failed to recognise the signs of kidney failure.

By the time he arrived in A&E in hospital he was very ill and later died. During his hospital stay he was robbed, subjected to aggressive patients and a lack of care from overstretched staff.

One example is a chamber pot under his bed had been knocked over and remained in that state for 2 days and until I insisted staff deal with it. This is not unusual and I have had many experiences of this when other members of my family have been hospitalised.

My brother being terminally ill and blind was left to fed himself in a hospital in Cambridge.

My mother was was about to be discharged from Trafford hospital fell and was transferred to Altrincham hospital against P&P and suffered a stroke which went unrecognised by new staff. She never recovered and died after days of a total lack of care weighing less than 5 stone.

I have worked as a manager in the NHS passionate to make a difference in mental health.

As you have identified a number of issues regarding your close family members that cover a number of services it is very difficult to respond to the specific concerns you have raised without being able to investigate in detail.
