What could be improved

so much room for improvement - for a start liaising doctor to doctor, and staff to staff on their change over - my mom was admitted with a suspected strok, only to find it was a LOW blood pressure problem. The dr she saw on AE immediately stopped the tablets she was on for HIGH blood pressure. She was admitted to the stroke ward - 2 days later they lost my moms medication sheet so put her back on the tablets she came in on - was totally disgusted, and let my feeling known to the staff and dr on call - instead of looking through the notes a nurse had just written down all the medications my mom came in on, hence she was put back on medication to lower an already low blood pressure - in this day and age this is totally unacceptable
