"No communication or information about diabetic clinic changes"

Diabetic. Self-caring and fully understands and manages her condition. On insulin. Used to attend diabetic clinic at hospital, now told it has transferred to Ashton Primary Care Centre. She has not been given any information as to what she should do, no appointment sent, and no explanation given. She always found the hospital clinic supportive and helpful, and feels disappointed and let down by a service she has used for many years.

Tameside & Glossop Clinical Commissioning Group decommissioned the Tameside Foundation Hospital Trust (THFT) Diabetes Outpatient Service in 2013 as part of the Clinical Care Group’s integrated services strategic plan. This is part of the NHS reconfiguration to move secondary care services back into the community. THFT consultants provide consultant clinics as part of that service and therefore, patients should see the same consultant as before where possible.

The service commenced in operation on Monday 3rd February 2014 and operates primarily from Ashton Primary Care Centre. This service replaced the Diabetes Services currently available from THFT and Tameside and Glossop Community Healthcare.

Any patients currently under the care of Tameside Hospital Foundation Trust or Tameside and Glossop Community Healthcare Diabetes Services automatically transferred into the Integrated Diabetes Service. A few patients were referred back to their GP for management of this condition following a review of the caseload undertaken by the Clinical Care Group

Services are now provided to those age 16 years and above, with a diagnosis of Diabetes, where the GP has exhausted all available treatment options within their sphere of experience.

It was planned that as part of the patient consultation all patients should have been written to prior to the service commencing. Please accept our apologies if you have not received the relevant information. You might want to inform your GP & T&G Clinical Care Group about the lack of information you have received as they also should have engaged with you as part of the patient consultation process. I hope you now have the correct information to support your health and wellbeing.
