"Doctor was not informed why I was attending"

About: The Princess Royal Hospital / Trauma and orthopaedics

(as the patient),

I attended an appointment with Mr Turner at PRH (MRI). He had not been informed why I was attending. He did not have MRI results.

The clinic was an arm clinic and not a neck clinic. Therefore why did this happen?
