"Fast track end of life continuing care"

Why is there going to be a wait of approximately 2weeks for short track assessment Consultant referral for fast track discharge because patient has rapidly deteriorating condition which is terminal and likely to be terminal in weeks not months. The patient wants to be at home with family all around. This is his right. The nhs states this. So why has there got to be a two day assessment when the consultants says it is fast track, I thought fast track meant automatically granted by nhs continuing care that's what it states in government papers., and nhs documents, so why an assessment waiting list. Patients wish is to die at home, 2 weeks may be too long a wait so patient will have to forgo his right to die at home. Disgusting at such a stressfull time for any family.
