(as the patient),

I was unwell, I usually have the district nurses visiting for a wound. Because I was unwell she did a thorough examination, took some blood spoke to the gp. Changed some equipment. Before this visit she had referred me to the Occupational Therapist to sort some other things out.

Hopefully when we have the Integrated Care teams set up and running in Mansfield & Ashfield, this Multi Disciplinary, Multi Agency approach will be the norm for patients who require different disciplines to attend to patient's needs.
