About: Telford referral and quality service (TRAQS)

(as the patient),

Went to doctors in October. Originally had a letter from TRAQS in November stating I had been referred by my doctor to Donnington Dermatology Clinic. I then had to wait for an appointment which was made for January 2014.

The suspected skin cancer was quickly diagnosed as only an ageing wart, but the itching skin condition I have been suffered with for some 9-12 months was examined and felt maybe a form of eczema or similar.

A letter to my doctor was produced for prescription of lotion to add to bath water, cream to wash with and moisturiser for application after bathing.

I have a follow up appointment in March.
