No visible leadership on ward. Call bells unanswered until timed out. Placed behind patient's pillow out of reach. O2 nasal catheters frequently not replaced when pt moved instead left oxygenating floor. Poor aseptic technique. Doctors' requests ignored by nurses- one example when Dr requested an extra pillow, was answered by registered nurse "we ain't got none" Hourly medication not administered for 6 hours despite frequent requests and emphasis by nurses that it should be given hourly. Water jugs not replenished despite requests (and no restriction on intake) red infected linen bag left in room for 48 hours. Commode uncleaned. a few staff were good, and when person initially admitted the team on then had been excellent but not seen again in a week. Adverse feedback forms left with contact but so far no response.
